Accident or Work Injury Claims: DD Form 2527 and TRICARE

If an overseas injury was an accident or happened at work, the DD Form 2642 asks for a second form, DD Form 2527. What it is and how to describe the injury.

Most routine visits are not tied to an accident. If yours was, the claim form asks for one extra document. Here is what the DD Form 2642 says.

Block 7 of the DD Form 2642 has two questions, each with a yes box and a no box:

  • Accident related?
  • Work related?

The form says to check the boxes to show whether the patient’s condition is accident related, work related, or both. A fall, a car accident or a sports injury is an accident. An injury that happened while working is work related.

If either answer is yes

The form says the patient is required to complete DD Form 2527, “Statement of Personal Injury - Possible Third Party Liability.” Its reminder list tells you to attach it to the claim if the condition is accident or work related. The form’s instructions say you can download DD Form 2527 at tricare.mil/forms.

The reason is that someone else may be responsible for the injury, and TRICARE asks for a statement so it can look into that.

Describe it in block 8a

Block 8a asks for the condition that required treatment. The form says to describe the condition, with examples such as a broken arm, appendicitis or an eye infection. If the condition is the result of an injury, report how it happened, for example “fell on stairs at work” or “car accident.” The form also tells you not to list the services performed, because they are on the itemized bill.

The rest of the claim is the same

An accident claim still needs the signed DD Form 2642, the itemized bill, a narrative description of the reason for care, and proof of payment. See the claim checklist and the block-by-block form guide.
